Guardian Angel Program

The goal of the Guardian Angel program is to increase communication and ultimately improve the quality of resident care. Every resident room is assigned one individual and one alternate back-up to act as a “Guardian Angel”. This person will be in frequent contact with the residents and family members to address potential issues.

The Guardian Angel will:

• Talk to the resident weekly at a minimum.
• Contact the family at least once a month.

The Guardian Angel team will also meet at least weekly to ensure participation and note program progress and issues. Should the resident or the family indicate a concern, a Resident/Family Concern form will be completed immediately and forwarded to the Administrator for follow-up.
